Cardiac Disease (CAD/HF) and DiabetesNHP has integrated with Optum Health Care Solutions (OHCS) to provide personal health management solutions across the spectrum of healthcare ranging from maintaining health to living with chronic illness for your patients with coronary artery disease (CAD), heart failure (HF) and/or diabetes. These programs are voluntary and available to all NHP eligible customers and designed to improve quality of life, promote selfmanagement and reduce uncessary hospitalizations for individuals with CAD, HF and Diabetes. They are led by specially trained registered nurses who will become health advocates to your patients. The registered nurses specialize in chronic diseases and will provide support to you and your patients who participate in the program. You continue to direct your patient’s treatment plan with the added support of a disease management team to reinforce your efforts to achieve optimal clinical outcomes and patient satisfactions. The program includes the following components:
High Risk Pregnancy/Neighborhood AdditionsNHP has partnered with Matria’s Maternalink Program to aid in the identification and management of pregnant women at risk for preterm delivery. This program is intended to complement the care your patient is receiving from you and is offered to all eligible pregnant members at no out-of-pocket expense. The focus of this program is to promote the development of lifelong healthy habits that will improve the quality of life for our members who have a chronic condition. The program is designed to complement your prescribed treatment plan and clinical decision-making. Through this program, our Care Coordinators will work to encourage, support and educate members according to your recommended plan of care. The program will provide feedback to you through summary reports of interactions with your patients. As a participant in the program, your patients will receive the following: • Confidential pregnancy risk assessments used to determine if there are any significant risk factors present, and also used to allow us to customize educational materials to specifically meet your patient's needs. You will receive a report documenting the information that your patient shared with us.
• Verbal education regarding such things as the
importance of good prenatal care and maintaining a healthy pregnancy.
• Toll-free access to our 24 hour, 7 days a week, BabyLine. This pregnancy information line is staffed by experienced obstetrical nurses and is available through six weeks postpartum. Those members identified as at high risk that could benefit from Case Management are also referred to NHP’s OB Case Management Department for ongoing monitoring and coordination of care.
